Add custom map (osmapa.pl)

Could someone help me add custom map?
I want to add maps from http://osmapa.pl. I try do it in many ways but i see only black tiles.
I thing that this should be possible because Locus for android use this map.

The map tiles have this format:

This results in this settings:
